While working on the physical rehabilitation unit (home unit):

• Correctly take and chart patient’s vitals, intake and outputs
• Report patient health and skin concerns to nurse
• Ambulate patients according to their physical assessment

While working on behavioral units:

• Correctly take and chart patient’s vitals, intake and outputs as well as behaviors
• Notify nurse of any signs of self-harm or increasing agitation amongst patients
