As Interim Choices Manager it is my responsibility to ensure that my team has the tools and knowledge to be successful in completing their daily responsibilities. This requires due diligence by having weekly checkins with staff as well as monthly meetings to address any concerns and to go over any new information. It is also my responsibility to step into any of the various roles as they arise. Additional responsibilities include working with other agencies to assist and educate them on requirements for the Choices Program, Attending Quarterly Meetings with TennCare to stay up to date on any new or future change's, submitting monthly and quarterly reports to Tenncare and reviewing monthly expenses and time sheets for the team.

• Assist Manager with being the first contact for staffing concerns
• Training new employees to be successful in their new role
• Stepping into any role as it arises
• Conducting Assessment's for clients within the 13 counties we serve
• Update or close out cases and update data basis
• Assign referrals as they come in
• Assisting clients with redeterminations as they come in from Tenncare
• Work with families if they have any complaints or concerns and to try and come up with a solution
• Work with outside agencies in assisting them and answering any questions they may have
Managed client caseload to ensure timely assistance to older adults and caregivers.
• Conduct home visits to assess client eligibility for various programs and services
• Completing Pre-Admission Evaluations for both home care or nursing home care as well as Medicaid
•Applications and collecting financial verifications as requested by Tenncare
• Keeping informed on Aging and Disability issues, and available resources and local service providers
• Being the liaison between families and Tenncare
• Other projects as requested by the Program Manager
